Our life journeys are personal and distinctive. Yet, we rarely go through them alone. If we are lucky, we find a community to join us, help us, and support us on our journey.

At Chase Brexton, implicit in our mission and vision is the goal to be that community to our patients. A community health center is a unique entity serving as, what many call, a safety net provider for those who may otherwise be unable to access care. However, it is our intention to not simply serve in cases of need but to provide as a community of care for the many lives we touch.

From our attendance **at more than 100 outreach events providing testing and education** to our Every Meal Matters food security program which provided recipes, education, and more than 99,000 pounds of food to more than 6,600 patients, our commitment to expanding access to care goes beyond medical care and into the everyday lives of those in the communities around us.

An expansion of our **free walk-in HIV testing, our new Sexual Health Clinic provides free walk-in STI testing and personal care items to those in our communities – in FY24, the clinic served 1,400 patients**.

**Our intensive** [**MyA1C InCheck diabetes management program**](/services/diabetes-management "Diabetes Management") brings together clinicians, pharmacists, and nutritionists to support those with uncontrolled diabetes, providing them tools that enable them to take control of their diabetes and drastically lower their A1C to healthier levels. The program is serving xxxx patients, xx percent of whom have healthy A1C levels.

**Addressing the need for earlier prenatal care and ensuring postpartum care is accessed**, our OB/GYN team developed an innovative program that provides patients who achieve their care goals with items to support them and their new baby. This remarkable program increased attendance to postpartum care visits by xxxx.

In FY24, we were astounded to provide **$13,075,746.19 in uncompensated care to our patients**. Additionally, we supported our patients with $86,644 to fill in financial gaps for housing, food, utilities, and medical expenses outside of Chase Brexton.

A community of care. What many may need on their life journey is here. Often, our patients are surprised by how much they deserve and will receive from this community called Chase Brexton. We are grateful to be a part of our communities and be a part of the lives of our patients. We hope they know they are deserving of exceptional care.
