At Chase Brexton Health Care, superheroes wear scrubs…and suits…and business casual…and sometimes awesome T-shirts. We are a community health center focused on providing outstanding patient-centered care that supports and inspires our patients to live their healthiest possible lives.
We are not your average healthcare center. As a 501(c)(3) non-profit primary care health center, we provide a range of clinical services from primary medical care and behavioral health services to dental and pharmacy, among others, and welcome more than 50,000 patients annually. From our medical team to our facilities team, we are united by the belief that healthcare can change lives.
Providing care that honors, affirms, and supports each patient is at the core of everything we do. Founded as a volunteer-run clinic, we started with a vision of meeting the needs of a community that was limited in their healthcare options. More than 40 years later, we’ve continued and expanded the effort: working to ensure everyone in every one of our communities has access to affirming, expansive, and individualized healthcare.
A Federally Qualified Health Center
Established in the 1960s, the Federally Qualified Health Center (FQHC) program is a vital source of care that focuses on the community and the individuals that comprise it. They recognize that where people live, work, worship and play affects their health, also known as non-clinical factors of health. They work to meet patients ‘where they are’ and give them the education and inspiration to achieve their goals.
FQHCs are given federal support to be able to provide care to those who cannot afford to pay by complying with both stringent quality and organizational guidelines:
- Qualify for funding under Section 330 of the Public Health Service Act (PHS).
- Qualify for enhanced reimbursement from Medicare and Medicaid*, as well as other benefits
- Serve an underserved area or population
- Offer a sliding fee scale
- Provide comprehensive services (either on-site or by arrangement with another provider), including:
- Preventive health services
- Dental services
- Mental health and substance abuse services
- Transportation services necessary for adequate patient care
- Hospital and specialty care
- Have an ongoing quality assurance program
- Have a governing board of directors of which at least 50% are patients

A Joint Commission Accredited Health Center
Accredited in Ambulatory and Behavioral Health, Chase Brexton meets (if not exceeds) the standards of the nation’s largest healthcare accreditor.
Accreditation is awarded after an unannounced, on-site survey assessing compliance with established standards. Joint Commission standards are patient centric and focus on organizational systems and processes essential to the delivery of safe, high-quality care. Standards are informed by evidence associated with structures and processes predictive of better care. They include patient rights and education, infection control, medication management, and preventing medical errors. Accreditation reviews occur roughly every three years.
A Patient-Centered Medical Home
Chase Brexton Health Care, an FTCA-deemed entity, is also accredited as a Patient Centered Medical Home (PCMH) through the National Committee for Quality Assurance (NCQA). PCMH is an approach to providing comprehensive and coordinated healthcare for patients of every age.
The focus of the PCMH model is creating the highest quality, safest, and most effective healthcare experience for each individual patient.
The PCMH model creates a health care setting that facilitates partnerships between individual patients and their personal primary care provider (PCP), and when appropriate, the patient’s family. The guiding principles of the PCMH model include:
- Patient-Centered: Care is relationship-based, respecting the patient's unique needs, culture, values, and preferences.
- Comprehensive: A team-based approach—including primary care providers, nurses, therapists, pharmacists, and case managers—is taken to meet the majority of physical and mental healthcare needs.
- Coordinated: Care is organized across the entire health system.
- Accessible: Offers shorter waiting times, enhanced in-person hours, and 24/7 electronic or telephone access.
- Committed to Quality: Practices use health IT and evidence-based medicine to improve care and patient safety.
