What is the Medicare Annual Wellness Visit? 

It’s a free yearly check-in for people with Medicare. This is not a physical exam. It’s a conversation with a Chase Brexton provider to help prevent illness and review health goals. Best of all – it can be done in person or by phone or video! 

During the visit, we'll go over your:

• current medications,
• recommended cancer and health screenings,
• and medical history.

We'll talk about:

• how you've been feeling,
• any health concerns,
• and issues you may be having with everyday activities.

And, we'll get you set up with:

• a personalized health plan,
• referrals to specialists,
• screenings and tests you might need,
• and medication refills.
