(11/1/22) In August 2021, Chase Brexton Health Care launched the Diabetes Management Program out of our Columbia Center. The program uses a multi-disciplinary approach focused on best practices in the field. It's personal and individualized, assisting patients in making disease management easier to understand, implement, and stick to.

Sarah Connor, DO, Family Medicine Physician (She/Her)
What is your favorite healthy snack?
Kind snack bars for on-the-go snacks; hummus and any vegetable I have (cucumber, carrots, celery).
What is your role with the Diabetes Management Program?
Lead physician (Diabetes Management Program Champion)
Why is this program important to participants? What is the impact?
As a primary care doctor, managing diabetes is our bread and butter. I have seen patients with devastating effects from uncontrolled diabetes, so I want to give them all the resources to prevent this. Our team provides the guidance, encouragement and tools to get the patients on track and in charge of their future. I often say, "easier said than done," as I want acknowledge the barriers and commitment it takes to control diabetes.
What else would like us to know about diabetes or the program?
After a very successful year of launching the diabetes program the Chase Brexton Columbia Center, we are excited to expand the program to the other Chase Brexton Centers in 2023!

What is your favorite healthy snack?
I love having a variety of flavors to munch on: sliced apples, nuts, and carrots.
What is your role with the Diabetes Management Program?
Registered dietitian. I help patients understand how foods and nutrition affect their bodies to successfully manage their diabetes.
Why is this program important to participants? What is the impact?
Food is important for us; it provides us essential nutrients and energy and is often the center of cultures, holidays, and celebrations. Living with diabetes may feel like it's hard to manage both our health and the connections we have with foods. Being in this program helps patients understand how to live a healthy life through food, medications, and movement. Each member of the team works very closely with patients, providing care that meets their individual goals and needs.
What else would like us to know about diabetes or the program?
Eating while living with diabetes does not mean you have to say goodbye to your favorite foods. When working with a dietitian, you will learn how best to incorporate your favorites in a healthy way, while achieving your health goals you and your team have set.

What is your role with the Diabetes Management Program?
I am a medication management specialist that helps patients get the most of their medications. I also act as a liaison between the pharmacy and the medical departments to help quickly troubleshoot and solve medication related problems.
Why is this program important to participants? What is the impact?
This program is important because all patients deserve to have their medication questions answered, why medications are prescribed, how they work, and what they should expect. The impact is that we can save a foot or someone’s vision; prevent a stroke, and improve their quality of life.
What else would like us to know about diabetes or the program?
You should know that we have a special program here at Chase Brexton to help underserved patients get affordable medication and diabetes supplies.
