Meet our two new Entry to Care Nurse Practitioners, who joined Chase Brexton in October 2025!
Chase Brexton Health Care is a busy place, and new patients sometimes wait weeks or even months to get in for their first visit. Enter: the Entry to Care Providers! They are poised to see new patients via telehealth within the coming week, and will gather medical, surgical, family health history, ensure a complete and accurate medication list is set up and ready to go, and address any urgent needs so the primary care provider who sees the patient later on is no longer starting from scratch.
We think this is a triple win--good for patients, good for providers, and good for Chase Brexton, too. Rashida and Amy are both Family Nurse Practitioners with a breadth of experience in Community Health Centers. Welcome, Rashida and Amy!
