Patients with diabetes who are seen at Chase Brexton’s Columbia Center will soon receive additional support and treatment thanks to a recent grant from the Maryland Community Health Resources Commission.
The two-year, $325,000 grant will expand Chase Brexton’s patient-centered model of chronic diabetes management. With the funding, Chase Brexton will expand its capacity to provide treatment and support services to Columbia Center patients with diabetes, including intensive care and treatment adherence support to those diagnosed with “uncontrolled” diabetes.
Columbia Center’s program offers comprehensive diabetes treatment, including health literacy around diabetes treatment, self-management skills, and health eating habits. With the funding, the program will also work to reduce diabetes-related health disparities for 90 low-income, underinsured or uninsured residents of Howard County, including racial and ethnic minorities.
