What is HOPE?

HOPE = Healthy Outcomes through Patient Empowerment

HOPE is a, GE-sponsored, team-oriented health care service available at Chase Brexton to help people take control of medical problems like high blood pressure, diabetes, and cholesterol. As a part of this unique service, your doctor, pharmacist, nurse, and other health providers will be working together closely to ensure you have the information, support, and medications you need for the best outcomes possible.

What can HOPE do for you?

The HOPE program will help you understand how to improve your overall health by:

  • Teaching you about your health conditions;
  • Giving you ideas and support for the best ways to take control of your health, including how to set (and achieve) personal goals;
  • Providing you access to support group sessions;
  • Helping you manage your prescription medications (pillboxes, refills, etc.).

 How do I become a part of HOPE?

  • Make an appointment for HOPE.
  • Bring all of your medications (prescription, over-the-counter, herbals, and vitamins) to your first HOPE appointment.
  • Now you have taken the first steps to controlling your health!
