This week, October 13-19, is National Case Management Week! Our case managers make the difference for their clients every day, providing needed support that removes barriers to a healthier life.
As part of our celebration of National Case Management Week, we wanted to introduce you to one of our amazing case managers, Kesh Coutain!
What made you want to be a case manager?
What made me want to be a Case Manager was the strong passion to speak up for individuals in my community that aren't necessarily heard. To be their advocate and ensure they receive the resources and support they need.
What have you learned during your time as a Chase Brexton case manager?
While at Chase Brexton, I've learned the ability to connect with the clients and meet them where they're at without judgement or prejudices. Clients come in and see I'm human too. I've been able to build trust with each of them which helps in effectively managing them through their barriers.
What excites you about the future?
What excites me about the future is seeing my clients thrive. Seeing those who struggled with coming to their appointment or taking their meds make that turn and be compliant. One of my toughest clients, a person I was unsure I was reaching, has successfully become undetectable, has stable housing and is a totally different person since our first meeting. The work we do here at Chase Brexton is sacred and so important to the community. We can never lose sight of that.
Thank you, thank you to ALL of our incredible case managers!
