National Health Center Week 2024

(8/2/2024) It’s National Health Center Week! Did you know Chase Brexton Health Care became a Federally Qualified Health Center (FQHC) in 1999? What is a FQHC exactly? FQHCs are community-based health care providers that are awarded funds from the Health Resources and Services Administration (HRSA) health center program to provide primary care services in under-served areas. 

The theme of National Health Center Week (NHCW) 2024 is “Powering Communities through Caring Connections.” At Chase Brexton, we strive to build and grow programs that are continuously serving and meeting the needs of our amazing community! Hear from a few of our incredible staff members on how their programs foster a caring community. 

The Diabetes Management Program

Clinical Pharmacy Specialist, Bri Murray, Pharm D, BCACD says, “Our program empowers our patients by giving them the tools they need to better manage their diabetes. Diabetes is not easy to live with and we support them along the way. We celebrate all the wins, big or small in our program.”

Kendall Pryor Smiling

Lead Certified Peer Recovery Specialist, Kendall Pryor, says, “As I have worked towards achieving sustained recovery and wellness, I've also learned to operate from the fruits of the spirit. This has also helped me to recognize the urgency of meeting the needs of the whole person from a holistic approach. Peer Services embraces the sensitivity needed to build empowering relationships of the individuals we serve at CBHC. And with that lives the opportunity to positively impact our communities. We've reached a particular climate in society where people are searching for love and acceptance. The lack of it is often perpetuated by violence, chronic illness, and literacy challenges. We, as a people, should be working towards change. And that's what CBHC is to the communities we serve."

Every Meal Matters

The Every Meal Matters (EMM) Program at Chase Brexton Health Care (CBHC) is a labor of love and collaboration made possible through partnership with The Maryland Food Bank and local volunteers. EMM is more than a food pantry. It provides our patients the food security and confidence to lead healthier lives. Paired with nutrition education and recipe ideas, patients leave their medical appointments with an arm full of groceries and peace of mind.

The POWER Project

Social Work and Outreach Department Program Manager, Tonya Williamson, CCHW says, “POWER Project empowers patients by allowing them to be active participants in both their healthcare and lifestyle choices.  We partner with the community to ensure there is access to needed resources.   We prioritize needs, ensure you leave knowing you have been seen and heard!

 Mobile Outreach Coordinator, Tikia Wilson, says, “For me, our empowerment is the community!  We listen, we act, and we are available.  An important part of our project is caring for those we meet.

Population Health

Population Health Team

Population Health Medical Director, Leah Hart, DrPH, FNP, says, “Population health works behind the scenes and between visits to make sure all clients are getting the care they need when they need it! 

The Navigators and Care Coordinators conduct outreach via phone calls and text messages to help patients get lifesaving care, like cancer screenings, and to ensure prompt follow up from the hospital. The pre-visit planning team preps client charts with notes and immunization records from outside Chase Brexton's system to help the primary care team serve clients effectively and efficiently. They also address social determinants of health by administering HEART funds under the MDPCP program, facilitating transport to medical appointments, and connecting patients with community resources using a powerful search engine called FindHelp.”
